Endacopia Surgeon: What He Built and His Ending C Role

The Surgeon made Clockey, RODGER and The Bride, and he is the figure Mellow sits down and questions inside a cave at the end of the Ending C route. Those two facts are the whole of what is solidly documented about him, and they are enough to make his article the largest on the official Endacopia Fandom at 38,449 bytes when we measured it through the Fandom API on 8 September 2026.

Content warning. Steam's own content descriptor for Endacopia reads: "This game contains scenes of body horror, psychological horror, distressing imagery, blood, violence, death, drugs, and jump-scares." The Surgeon sits at the centre of the body horror strand, and the community wiki attaches its own content warnings to the closely related article on The Bride.
The name is not a name
The community wiki has no personal name for him. It has a redirect, and the redirect is informative: an article once existed under Freelance Surgeon, and the editor who folded it into The Surgeon wrote as their reason that this is his occupation, not his name. That is the same housekeeping logic the wiki applied to The Cowboy, whose deprecated title The Sharpshooter was retired because being a sharpshooter is an occupation.
So the canonical form is The Surgeon. No source we can check gives him a birth name, an age, a species or an origin. If you find a page that does, it is inventing.
What the Surgeon made
This is the strongest claim on the page, and the reason it is strong is worth stating plainly: it is not one sentence on one article. Three separate character pages on the official Fandom point back to the Surgeon as their maker, and they do not contradict each other about it. Cross-page consistency inside a wiki that is edited by different people is real evidence, in a way that a single unsourced line is not.

Two naming traps in that table are worth defusing, because both cost readers time.
RODGER is not Rodger. The all-caps RODGER is the copy. Rodger Thompson is the real name of the player character, Mellow. The wiki lists Fake Mellow, Mirror Mellow and Mimic as aliases for RODGER and marks all three as unofficial, so use RODGER when you search.
Clockey is not Clocky. The wiki holds redirects from Clocky, Clocky 2 and Ticked Off Clockey, with one editor's note recording that the misspelling was the reason for the cleanup and another noting that the "ticked off" form spoils the ending in the title. Clockey is the only spelling that will find you a live article. He is the one boss on the boss list that a first playthrough cannot skip.
Our dossier also records the wiki stating that Mellow is killed by Henry and brought back by the Surgeon. We mark that single-source: it is one page's plain text, not a claim we have seen corroborated across several articles the way the three creations are. It is plausible, it is on-theme, and it is not yet at the confidence level of the table above.
The Surgeon's role in Ending C
Ending C is the reason most people search for this character. He is the payload at the end of it: not a fight, not a puzzle, but a figure who explains things. The wiki's article on him carries a dedicated section headed "Role in ending C", which is unusual precision for this wiki.

cheshire is the only code in Endacopia that we can verify. There are no safe codes, no numeric vault combinations and no physical pass that opens Ending C. Lists offering a different name for Ending C or a collectable item as its key are not describing this game.
The questions in the cave
This is where we have to stop, and stopping is the point of this page.
What is documented is the shape of the scene: Mellow sits, Mellow asks, the Surgeon answers, and the answers are the game's explanation of itself. What is not documented anywhere we can check is the content: the individual prompts, how many there are, whether the order matters, whether any of them can be missed, and whether the ending triggers after a set number. No source publishes that list. We are not going to reconstruct it from memory or from vibes, because a reconstructed dialogue tree is exactly the kind of thing that gets copied across five aggregator sites and becomes folklore.
The same limit applies to his motives. Our rule on this page is that we report what the wiki states in plain text and nothing past it. The wiki explains that he built three things and that he explains the lore. It does not, in any passage we verified, give a stated goal, a plan or a justification. Everything you have read about why he operates, what he wants with Mellow, or what he represents is a reading, not a record. Some of those readings are good. The community's fondness for the surgeon-as-author idea, given that the whole game takes place inside a body, is an elegant one. It is still interpretation, and it belongs on a meaning page, labelled as such, not in a facts table. The wiki itself is honest about this habit and writes "this is just speculation" in several places, which is more than any competing Endacopia site does.

The first row is the live argument in this community. Note that the strongest wording, "requires ... first", comes from the source with the least to back it, while the site closest to the game's own editors is silent on it. Note also the direction of the second row: the Fandom is explicit, and the sites contradicting it are the ones with no citations. We give the Fandom more weight and still flag the conflict.

Endacopia has 16 Steam achievements. Steam hides the descriptions of four of them; the official Fandom decodes all four, and the names line up with the ending letters across four independent sources.

Those percentages are live figures pulled from Steam's global stats on 8 September 2026 and they will drift. The gradient itself is the interesting part: it falls exactly in the order the community describes the endings as being reached, which independently supports the ending-to-achievement mapping. What no source documents
Stated plainly, so you know where the record actually stops:
- No personal name. Only the occupational title.
- No stated motive, beyond the fact that he built three characters and explains the lore when asked.
- No question list for the cave scene.
- No confirmed relationship to God, the entity the game takes place inside, or to any other figure. Any such link is a reading.
- No Chapter III walkthrough anywhere. The official Fandom's Chapter III article is three bytes long and reads "tba". Everything we can say about Chapter III comes from the endings articles working backwards.
- No verified transcript. The Fandom cites a recorded Surgeon question-and-answer video as a reference. We have not opened or verified it, so we do not quote from it and do not list it as a source here.
Two corrections while we are in the neighbourhood, since both errors circulate on pages about this character. The hub area connecting the three regions is called "the green place" on the Fandom, not "Green Hills". And Misery Town is circus and carnival themed, full of clowns, mimes and a whack-a-clown game, not a decaying suburb; that error is trivially disproved by any screenshot.

The Surgeon is the figure who made Clockey, RODGER and The Bride, and the character Mellow can sit and question inside a cave during Ending C. The community wiki gives him no personal name. An older article titled Freelance Surgeon was redirected to The Surgeon by an editor whose stated reason was that being a surgeon is his occupation, not his name.
What questions can you ask the Surgeon in Ending C?
The community wiki records that after crawling into his cave and reading his book, Mellow sits down and puts questions to the Surgeon, and that his answers explain the game's underlying lore. Anyone presenting a numbered set of Ending C questions is going beyond what any checkable source says.
Do you have to finish Ending A and Ending B before Ending C?
The sources disagree and we will not pick a winner. The official Fandom does not list A and B as prerequisites at all and simply gives the Ending C route from the Prologue onward. A Steam Community guide says Ending C is intended only if you have completed both, which is advice rather than a hard gate. A third fan site states outright that both must be unlocked first. Treat finishing A and B as a strong recommendation with contested status.
Is the Surgeon a boss fight?
No source describes a fight with the Surgeon. He appears in the Ending C sequence as a figure to read from and talk to, not to punch. The boss that is confirmed in that same room, Mellow's Room, is Clockey, encountered a second time in a state the wiki calls ticked off; the Clockey article's infobox lists his death place as Mellow's Room, Chapter 3, Ending C.
Did the Surgeon bring Mellow back?

That is one page's plain text rather than a claim confirmed across several articles, so we mark it single-source. Can I play Endacopia free in a browser to see the Surgeon scene?
No. Steam's own store data for app 2684630 lists Endacopia at $16.50 on Windows only, with no free edition and no browser build. Sites advertising a free in-browser version of Endacopia are wrong, and their claim is contradicted by first-party Steam data. There is no legitimate way to reach the Ending C cave except by owning and playing the game.
